Factors such as where a person lives, their financial stability and education level, their access to food and more are known as \u201csocial determinants of health.\u201d Priority Partners and Johns Hopkins Health Plans are committed to helping members and neighbors overcome these barriers through community action.&nbsp;<\/p>\n\n\n\n<p class=\"has-h-3-font-size wp-block-paragraph\"><strong>In Your Community<\/strong>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Visit our <a href=\"https:\/\/ppmco.foxtrotdev.com\/community-health-advocates\">Community Support page<\/a> to learn about these and other events.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>35th Annual Ocean City Community Health Fair<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Date: April 2, 2024<\/li>\n\n\n\n<li>Location: Roland E.
